發病機制與病理成因拆解: 自體免疫、氧化應激與遺傳誘因
白蝕症的發病機制錯綜複雜,目前醫學界普遍將其歸因於遺傳易感性、自體免疫異常、細胞氧化應激及神經體液因素共同作用的結果1。了解這些底層機制對於理解現代標靶藥物的作用原理至關重要。
細胞毒性T淋巴細胞(CD8+ T細胞)介導的自體免疫反應是黑素細胞死亡的核心驅動因素1。當患者體內的免疫耐受受損時,激活的CD8+ T細胞會特異性地識別黑素細胞表面抗原並釋放干擾素-γ(IFN-γ)。IFN-γ結合細胞表面受體後,會激活細胞內的Janus激酶(JAK1/JAK2)及信號轉導與轉錄激活因子(STAT)通路,進一步誘導皮膚角質形成細胞分泌趨化因子CXCL9與CXCL10。這些趨化因子會持續招募更多T細胞集聚至病灶,形成自我維持的自體免疫炎症循環,最終導致黑素細胞的大量凋亡4。

除了免疫因素,黑素細胞內部的氧化應激不平衡亦扮演了重要角色1。黑素合成過程本身會產生大量活性氧(ROS),若黑素細胞內部的抗氧化酶系統存在先天缺陷,過量的ROS蓄積將破壞內質網功能,引發細胞應激反應並釋放損害相關分子模式(DAMPs),進一步觸發局部免疫系統的反撲1。
遺傳學研究顯示,約有三分之一的白蝕症患者具有家族遺傳背景2。同時,白蝕症患者併發其他自體免疫疾病的風險顯著高於常人,其中約有三分之一的患者同時伴有甲狀腺功能亢進或低下(如橋本氏甲狀腺炎),其他常見關聯疾病包括斑禿、一型糖尿病及類風濕性關節炎等2。 臨床上也常見到同形反應(Koebner Phenomenon),即皮膚在遭受刮傷、曬傷、摩擦或手術切口後,創傷部位出現新的白蝕病灶,這提示物理性損傷可直接誘發局部的免疫炎症反應。
